Case Manager within a collaborative program designed to assist in removing social and emotional challenges for children experiencing school attendance and/or behavioral issues. Additional responsibilities include the coordination of a mentorship program. Requires a bachelor’s degree in social work or related human service degree, knowledge of family centered case management, social work principles, and related experience to effectively work with African American families and communities. Must possess effective communication skills with clients and colleagues, computer literacy, valid MN Driver’s license, and dependable transportation. Persons of color encouraged to apply.
